What about permission slips? A little advice and a sample.

I am often asked, "Do you have a permission slip that you can recommend? Are they legally binding?"

The answer is that there is no such thing as a legally binding permission slip, because they have never held up in court, so it's really up to the doctor to decide in an emergency whether he is willing to risk treatment.

However, the better the permission slip looks, and the better the clear language of what the parents are granting permission for, the better the chances that a child will receive good and timely care.
